Need Help

 

         CAP helps any Putnam County resident who has a cancer diagnosis and is receiving treatment. The help is in the
         form of monetary support. CAP reimburses for mileage for treatments and any tests or MD appointments that are
         associated with the cancer treatment. The forms must have a signature from the physicians on the initial form.
         This form then needs to be resigned by the physician at the beginning of each year. The forms should be submitted
         to the address on the form at least quarterly. They are processed monthly at the end of each month. There is no
         income guidelines and there is a guideline form to follow. CAP will reimburse for the year up to March of the following
         year. All forms need to be submitted by March.

         Donations
         CAP accepts donations at any time. Donations have come in the memory of someone, in honor of anniversaries,
         birthdays, retirement, etc. Checks can be made out to CAP.  Donation Form
